I've spent my career moving between two worlds — the hospital and the codebase — because I believe the most important problems happen at their intersection.

I'm Dr. Tarvinder Singh, a board-certified vascular neurologist and the Co-Founder & CEO of FIRY AI, where we're building AI-powered tools to transform neurological care. Before medicine, I spent years as a software architect at Microsoft, building systems at scale.

I serve as Chief of Neurosciences and Chief of Neurology at Providence, where I see firsthand how AI can — and cannot — improve patient outcomes. This dual perspective drives everything I do: I understand what clinicians actually need because I am one, and I understand what technology can realistically deliver because I've built it.

What I'm Building

At FIRY AI, we're developing AI-powered neurological care tools — starting with headache assessment — that put sophisticated diagnostic reasoning into the hands of patients and providers. Our mission is to make expert-level neurological care accessible to everyone, everywhere.

Here, on this site, I'm building three things:

  • A book on the future of AI in medicine — written in the open, free to read, published chapter by chapter as I refine the ideas.
  • Essays and articles — examining the real challenges and opportunities at the intersection of AI, neuroscience, and clinical practice.
  • This website itself — an experiment in autonomous AI-driven development, where AI agents propose, plan, and build new features every day, under my review.

Three Principles

Everything I build is guided by these convictions:

  1. Augmentation over Replacement — AI should amplify human clinical judgment, not replace it. The best outcomes emerge from human-machine collaboration.
  2. Transparency over Opacity — Any AI system that influences medical decisions must be explainable. Black-box medicine is not medicine.
  3. Equity over Efficiency — If a technology only benefits those who already have access, it has failed. AI must reduce disparities, not encode them.
